- Request confidential communications: Ask us to contact you in a specific way (for example, at a certain phone number or mailing address).
- Receive an accounting of disclosures: Request a list of certain disclosures of your information made for reasons other than treatment, payment, and health care operations.
- Get a paper copy of the Notice: Request a paper copy of our full Notice of Privacy Practices at any time, even if you have viewed it electronically.
- Be notified of a breach: Receive notification if a breach occurs that may have compromised the privacy or security of your protected health information, as required by law.
Our Responsibilities
We are required to:
- Maintain the privacy and security of your protected health information.
- Provide you with a Notice of our legal duties and privacy practices.
- Follow the terms of the Notice that is currently in effect.
- Inform you if we change our privacy practices and make the revised Notice available.
